Assisted Living Facilities Contracted with Trelles PharmacyContact Joy Cooper at 813-846-9650 or 813-662-1153, ext. 212 to schedule a Cart and MOR Review at no cost to you.Assisted Living Facilities not contracted with Trelles PharmacyContact Joy Cooper at 813-846-9650 or 813-662-1153, ext. 212 to schedule a Cart and MOR Review for the fee described below.Facilities with 3-30 residents on med service - $499Facilities with 31-75 residents on med service - $599Facilities over 75 residents on med service - $700Payment may be made by credit card (VISA, Mastercard, American Express or Discover) or by check (made payable to Trelles Pharmacy). Payment is required no later than 24 hours prior to the day of service.
